From referral to report.

  1. Send the referral

    Use the form, WhatsApp, or email a referral letter. Include the diagnosis and any precautions.

  2. We contact the patient

    We call the patient or their family to explain what to expect and book an assessment.

  3. Assessment and plan

    A therapist assesses the patient at our Parklands clinic or at home and agrees goals with them.

  4. We report back

    With the patient’s consent, we send you a short summary of the assessment and the plan.
